Navigate to consentcompanion.app and sign in

1. Setting Profile Information
  • Tap the avatar icon in the top right of the header to navigate to the profile screen.
  • Find the Profile Information section and press the "Expand" option.
  • The First name, Last name, and the Practition License# are required to automate the consent form pdf.
  • Fill this form out and click the save button underneath.
2. Choose your settings
  • Tap the avatar icon in the top right of the header to navigate to the profile screen.
  • Find the Application Settings section and press the "Expand" option
  • The minimum age on consent form sets the form to only accept clients older than this setting.
  • Choose preference on adding ink. "Add all inks" includes your entire ink list automatically, while "select inks" allows individual selection during consent form approval.
  • The "Compare Avg. Stats" feature allows you to opt in or out of comparing your form count with the average of all other app users.
  • The personal preference form is an optional addition to the consent form, designed to capture the client's comfort preferences for the session.
3. Add inks to profile (if applicable)
  • Tap the third icon (feather in ink) on the bottom navigation bar to access the ink manager screen.
  • Add inks individually by entering details like color, brand, and lot number, then press the "Save" button.
  • Inks added here will either be automatically attached to the consent form or prompt an ink selection during consent form approval, depending on your application settings.
  • Clicking the trash icon next to a listed ink will move it to "Old Inks," allowing you to continue searching consent forms by that specific ink.
  • Toggling the "Manage Old Inks" checkbox allows you to view or delete old inks. Deletions made here are permanent.
4. Add a client login (optional)
  • Tap the avatar icon in the top right of the header to navigate to the profile screen.
  • Find the Client Device Account and press the "Expand" option.
  • Enter an email and password in the fields, then click the "Create Account" button.
  • Log in with those credentials to restrict permissions to submitting consent forms only.
  • Changes made to settings from your main account apply to this login. If this account is active when changes are made, please refresh the screen to apply the new settings.
  • Once a client login is created, you can safely delete and create new ones at any time without losing any data, as the data is linked to your main account.
5. Completing a Consent Form
  • Tap the first icon (document) on the bottom navigation bar to access the Consent Form screen.
  • Select the procedure to continue with the form.
  • All fields, except for the middle name, are required for submission.
  • The client must upload or photograph the front side of a valid identification card.
  • If a date less than 18 years ago is entered, guardian requirements will automatically appear in the form.
  • If a date younger than your minimum age setting is entered, the form submission will not be possible.
  • The personal form will be displayed here if your application settings permit it.
  • The consent for photographing the tattoo and using it in a portfolio or showcase is displayed by default when the procedure is for a tattoo only.
  • The form requires one or two electronic signatures, depending on whether the client is a minor.
  • Once the form is saved, it will be available in the list of forms on the Approve Forms screen.
6. Approving a Consent Form
  • Tap the second icon (clipboard) on the bottom navigation bar to access the Approve Form screen.
  • The list of forms ready for approval, organized by the client's name are shown here.
  • Tap on any form to view its details on the page.
  • Verify the client's information manually.
  • If your ink settings require it, you may need to select the ink used before submitting. Tap the ink selection button and choose the inks used, if applicable.
  • Take any notes about this session, such as allergy's, medications, etc.
  • Electronically sign this document to approve and generate the PDF and save to the database.
7. Searching the Form Archives
  • On the bottom navigation, tap the fourth icon (storage container) on the bottom navigation bar to access the Archives screen.
  • Search through all consent forms approved by your account.
  • Utilize any of the following search criteria individually or in combination:
    • Client Email
    • Client Last Name
    • Ink or old Ink used
    • Month and Year
  • Once the desired form is located, tap on it to view client details, access a PDF link, or delete the form.
